- Full name (as it appears on the passport)
- Address while in the US
- Primary contact phone number and secondary or emergency contact phone number (*2 phone numbers required)
- E-mail address
- Please provide this contact tracing information prior to your departure date, through the JAL Web site, your nearest JAL office, or the JAL reservation center.
Through the JAL website, please register your contact phone number for Flight notification for each passenger. - Passengers traveling with a package or group tour should submit this information to the applicable travel agent. Contact your travel agent for more details.
